What Is Chronic Care Management (CCM) at Gateway Gastroenterology?

Gateway Gastroenterology is proud to offer the MyCare Program. This is a cutting-edge Chronic Care Management (CCM) service that ensures continuity of care for Medicare patients with two or more chronic conditions (at least one of which must be GI-related) by providing an ongoing relationship with a Virtual Care Team. While enrolled, patients can expect to have an individualized comprehensive care plan, interactive remote communication, assistance with medication management, and coordination of care between providers. The goal of CCM is to provide the best possible care by keeping patients with chronic diseases out of the hospital and improving their overall quality of life.

 

What CCM Services Does Gateway Gastroenterology Provide?

Patients will have access to a list of resources including a dedicated MyCare Coordinator, a Registered Dietitian, and a Patient Experience Team. We are introducing the MyCare Program for GI diseases such as Ulcerative colitis, Crohn’s disease, GERD, and chronic liver diseases like NASH, NAFLD, Obesity, Metabolic Syndrome and related conditions.



Do I Need a Referral to Receive CCM Services?

No! If you are interested in these services, all you have to do is call a MyCare Coordinator at 314-597-9748 to learn more and confirm that you qualify – it’s that simple!


What Should I Expect While Enrolled in CCM?

Each patient will be assigned a dedicated MyCare Coordinator. There will be patient education about your specific chronic gastrointestinal conditions or liver disease. You will have coaching on improving your diet, exercise, and motivation. There will be regular phone calls and emails to keep you accountable and on track. Remote Patient Monitoring (RPM) devices, such as a digital scale, may also be provided to track and measure your progress.

 

What Are the Benefits of CCM?

Benefits include increased access to appropriate medical resources, enhanced communication with members of your care team, reduction in emergency room visits and hospitalization or readmissions, and increased engagement and self-awareness of your health. For your convenience, patients do not have to come to the office to enroll in this program. It is offered through our telehealth platform.


Who is Eligible for CCM at Gateway?

Any Medicare patient with at least two qualifying chronic conditions that are expected to last at least 12 months, or if the chronic health conditions put them at significant risk of death, acute exacerbation, or functional decline, are eligible.


How Do I Enroll in CCM with Gateway Gastroenterology?

Please call 314-597-9748 during office hours (8:00am-4:00pm CST) Monday through Friday and ask to speak with a MyCare Coordinator. Enrollment in the MyCare Program is very simple and should take only a few minutes!
